Registrations are accepted on a first-come, first-serve basis.

EARLY REGISTRATION FOR  PLAYERS/CHEERLEADERS ARE ACCEPTED

JUNE 1, 2008 TO JULY 15, 2008

EARLY REGISTRATION FEE: $65.00

REQUIRED FOR NEW AND RETURNING PARTICIPANTS:

*1: Completed  Pop Warner Registration Forms.
*2: Completed Broncos Registration Form.
*3: Parents must purchase practice pants, 7piece pad, practice jersey and cleats for their child
*4: Photo.

*5: Birth Certificate.
*6: Game uniforms must be returned after each game.
6: Completed Medical Release Form with Dr. signature.
7: Copy of June 2008 Report Card (will kept confidential) MANDATORY

* These must be submitted at the time of registration.

# Medical Form and Report Card are due July 1, 2008
 

Players/Cheerleaders who have not submitted all paperwork will not be allowed to practice August 1st until required paperwork is submitted.

PRACTICE STARTS:
AUGUST 1st, 2008 6:00PM
All players should be at Fireside Rec Center 8601 Fireside Dr Dallas, Tx at 6:00PM on time.

Practice will be Monday, Tuesday, and Thursday, TIME 6:00PM to 8:00PM.
